RECOVER-VITAL’s August 31, 2026 report found no significant benefit on primary outcomes from 15 or 25 days of nirmatrelvir–ritonavir for established Long COVID. Analysis included 959 adults at 69 US sites, grouped by cognitive, autonomic or exercise-related symptoms. [1]

The question followed earlier disappointment. The 2024 STOP-PASC randomized trial found no improvement in selected symptoms with a 15-day course. Its authors identified small size, symptom variation and prolonged illness as reasons further targeted research remained necessary. A negative result had narrowed the evidence without settling every biological explanation. [2]

NIH’s September account compares extended treatment with ritonavir plus placebo. Symptoms improved at similar rates across groups: improvement during treatment alone cannot show that the antiviral caused it. [3]

Enrollment did not require evidence of persistent virus, and participants were predominantly White. Whether people with demonstrated persistence might respond differently remained unanswered; planned blood-sample analyses were separate from the clinical result. [1]

NIH placed this trial within RECOVER and said further analyses were planned. It tested established symptoms with specific regimens. It did not show symptoms were unreal or every potential treatment had failed: research still needed to connect proposed mechanisms to measurable benefit. [3]
